LIABILITY WAIVER
I understand that Team Elite Physique LLC's Contest Prep Center (CPC) is an advanced training facility and that all equipment should be used with full understanding of the function and operation of each piece of equipment. I also understand that the use of all equipment is done at my own risk and any injury or illness I sustain is not the responsibility of Team Elite Physique LLC, Adam Bonilla or the Contest Prep Center.
I understand that the CPC is a private training facility and trainers training out of the CPC are independent contractors (IC). I understand that if choose to hire a CPC IC trainer It is my sole responsibility to verify that they have adequate insurance, education and experience to handle me as a client. I further understand that any injury occurring from my personal training sessions will be handled directly with my chosen trainer and not involve the CPC, Team Elite Physique LLC. or Adam Bonilla in any capacity and release the CPC, Team Elite Physique LLC. and Adam Bonilla from any and all responsibility.
